Holyoke Health Center Chosen for Point32Health Quality Grant Program

“Holyoke, Mass. (July 19, 2022) — Point32Health has awarded Holyoke Health Center a Quality Grant for $89,900 to carry out a quality improvement project to ensure that all patients at the health center receive assistance with social and environmental factors that can negatively impact their health outcomes.

“Food insecurity, unsafe housing, lack of transportation, and poverty can dramatically affect a patient’s ability to maintain their health,” said Jay Breines, Chief Executive Officer at Holyoke Health Center. “As a community-based health center, our medical teams focus not just on symptoms, but on underlying lifestyle issues that can cause poor health. This Quality Grant from Point32Health will improve our ability to identify and assist patients who need support around these factors.”

The Quality Grant Program was established more than 21 years ago at Harvard Pilgrim Health Care, and this tradition continues in 2022 under the Point32Health banner following the combination of heritage Harvard Pilgrim Health Care and Tufts Health Plan networks last year. Since its inception, the Quality Grant program has funded more than 300 grants totaling more than $22 million of support to improve care delivery and reduce health care costs.

Holyoke Health Center, established in 1971, is a Federally Qualified Health Center providing affordable, community-based primary care, dental, pharmaceutical, vision, behavioral health, and other supportive health services to a primarily Latino, low-income community.
