Medical Records

The Health Insurance Portability and Accountability Act of 1996 (HIPPA), gives you rights over your healthy information, including the right to get a copy of your information, make sure it is correct, and know who has seen it. You may inspect the records or have copies made for a reasonable fee after signing an authorization. The Holyoke Health Center, Inc. is happy to provide you with a copy of your medical or dental record during our medical record department’s regular hours of operation.

Holyoke Health Center, Inc. respects your privacy, if you would like us to forward a copy of your medical record or dental record to a healthcare provider, we need your written authorization along with a legible copy of your valid photo ID. If you pick up your medical or dental record in person, you will be required to show a valid photo ID.

The Medical Records Dept. is located in the Lower Level of our Holyoke Health Center location at 230 Maple St. Our Chicopee Health Center Medical Record Dept. is located to the left of the Pharmacy in the main lobby.

There is NO FEE if we send the records directly to another healthcare provider. There is A FEE for copies of medical records you pick up or those which are mailed directly to you. This fee is legislated by the State of Massachusetts. Payment for copies can be made by money order made out to Holyoke Health Center, Inc.

Consideration will be given to waive the fees for patient in cases of financial hardship. In most cases, your copies must be given to you within 30 days.

Holyoke Health Center, Inc. has a patient portal that does make some of your medical information available for your review. If you have not enrolled, you can click on the Patient Portal link and follow the steps to self-enroll.
