Care Management
Our Care Management Department is a team of nurses, social workers, community health workers (CHWs), and care coordinators, who work with the patient’s primary care provider and medical team to support patients in meeting their medical, behavioral and social needs. The Care Management team provides programs such as Medicare Chronic Care Management, Complex Care Management, and connections to community resources for housing, transportation and food assistance. Our team helps patients in developing a individualized care plan, as well as supporting patients via telephone, in-person appointments at the health center, in the community or at the patient’s home.

The Care Management Department is located in the lower level of the Holyoke Health Center, and in the medical department of the Chicopee Health Center. To enroll in our care management program, please ask a member of your primary care team during your medical appointment or call (413) 420 – 2200 and ask to speak to a member of our Care Management Team.
