The Health Center
Holyoke Health serves more than 24,000 patients a year from communities throughout the Pioneer Valley. We have a committed staff of over 350, including 30 full-time medical and dental providers, 30 nurses, 50 bilingual medical and dental assistants, case managers, outreach workers, administrators, and financial counselors. We also have Community Health Workers who can answer questions outside of a doctor’s visit.
Because we are a federally funded non-profit community health center, no one will be denied access to services due to inability to pay; and there is a discounted/sliding fee schedule available based on family size and income. Our caring, professional medical and dental teams know that healthy families make healthy communities and we are committed to both.
Holyoke Health Center, Inc. is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
History
Holyoke Health has played an instrumental role in the communities of Holyoke, Chicopee, and the surrounding towns since its founding in 1970. Holyoke Health Center is a non-profit, Level 3 Patient-Centered Medical Home, and a Federally-Qualified Community Health Center (FQCHC).
Holyoke Health has a long history of providing comprehensive care as the only free-standing health center in the metropolitan area. The Center works closely with government agencies, private companies, and community members to ensure that every patient who seeks care is served regardless of their ability to pay. The Center was founded on the principle that everyone deserves high-quality healthcare services. We have delivered on that promise to 24,000 people a year with a continually growing list of services and providers.
Who We Are
At Holyoke Health, all of our Primary Care Providers are residency-trained and board certified. Our Medical Team includes family practice physicians, internists, pediatricians, nurse practitioners, pharmacists, pharmacy technicians, registered nurses, chronic disease nurse case managers, medical assistants, licensed dieticians, certified diabetes educators, HIV counselors and case managers, certified lactation consultants, and patient navigators. We have specialists on-site including obstetricians, gynecologists, cardiologists, infectious disease specialists, and behavior health specialists.
Our Dental Team includes dentists, oral surgeons, orthodontists, dental hygienists, dental assistants, and dental educators, along with support staff who provide both adult and pediatric oral health services.
