QUALITY AND ACCREDITATIONS
At Holyoke Health Center, our goal is to be the healthcare provider of choice for our community. We are dedicated to delivering high-quality, safe, and effective care to all our patients.
To ensure we maintain the highest standards, we seek accreditation from several nationally-recognized organizations. This involves regular on-site inspections of our health center locations to ensure safety and compliance. We also complete thorough reviews of our patients’ health outcomes, satisfaction and experience metrics. Additionally, the qualifications and training of our providers and staff are carefully evaluated.
Our commitment to ongoing compliance and quality improvement means that our patients can trust in the care they receive. We are proud to participate in this rigorous process, knowing it benefits our community’s health and well-being.
Our Accreditations
Holyoke Health Center is proud to be accredited by the Joint Commission, an independent, non-profit organization that certifies over 20,000 health care organizations in the U.S. By meeting the Joint Commission’s nationally recognized standards for safety and quality, Holyoke Health Center has earned the prestigious Gold Seal of Approval.
Holyoke Health Center is recognized as a Patient-Centered Medical Home by the National Committee for Quality Assurance (NCQA). The PCMH model is a team-based approach that puts the patient in the center of the health care team to support the patient in all of their healthcare needs.
FTCA FSHCAA Deemed Health Center
The Health Resources and Services Administration (HRSA) provides equitable health care to the nation’s highest-need communities. Their programs support people with low incomes, people with HIV, pregnant people, children, parents, rural communities, transplant patients, and the health workforce.
Holyoke Health Center receives HRSA Health Center Program grant funding under 42 U.S.C. § 254b and has been deemed a Public Health Service employee for purposes of certain liability protections, including Federal Tort Claims Act coverage, under 42 U.S.C. § 233(g)-(n).
HRSA Community Health Center Quality Recognition (CHQR) Badges
CHQR badges are given to Health Center Program awardees and look-alikes (LALs) for their significant accomplishments in areas such as access to care, quality, health equity, health information technology, screening for social risk factors, and responding to the COVID-19 public health emergency. These achievements are based on the most recent Uniform Data System (UDS) data.
Health Center Quality Leader (HCQL) Bronze (2022) badge, are awarded to health centers that have the best overall CQM (Clinical Quality Measures) performance based on average Adjusted Quartile Rankings (AQR). Bronze badges are awarded for being in the top 21-30% AQR ranking.
Addressing Social Risk Factors (2023) badge is awarded to health centers that:
1. Collect data on patient social risk factors.
2. Increase the percentage of patients who received enabling services between the last two UDS reporting years.
Advancing Health Information Technology (HIT) for Quality (2023) badge is awarded to health centers that:
1. Adopted an electronic health record (EHR) system.
2. Offered telehealth services.
3. Exchanged clinical information online with key providers’ health care settings.
4. Engaged patients through health IT.
5. Collected data on patent social risk factors.
Patient Centered Medical Home (PCMH) recognition has become a standard of care for HRSA funded health centers.
